Journal Article Summary

The article investigates the effectiveness of two medications, naproxen and loratadine, in preventing bone pain associated with pegfilgrastim in women undergoing chemotherapy for early-stage breast cancer. Bone pain is a common side effect of pegfilgrastim, which is used to reduce the risk of febrile neutropenia, a serious condition that can arise from chemotherapy. Managing this pain is crucial, as severe discomfort may lead patients to refuse pegfilgrastim, potentially increasing their risk of infections and negatively impacting their cancer treatment outcomes.

In this study, 600 women with newly diagnosed stage I–III breast cancer were randomly assigned to receive either naproxen, loratadine, or no prophylactic treatment while undergoing chemotherapy with pegfilgrastim. The primary focus was on the incidence of all-grade bone pain during the first cycle of treatment. Results showed that the percentage of patients reporting bone pain was slightly lower in the naproxen (40.3%) and loratadine (42.5%) groups compared to the no treatment group (46.6%), but these differences were not statistically significant. However, patient-reported outcomes indicated that both naproxen and loratadine were associated with lower levels of bone pain compared to no prophylaxis, with loratadine showing fewer treatment-related adverse events.

The study has some limitations, including its open-label design and the lack of a placebo group for comparison. Additionally, patients were allowed to take other medications for pain relief, which may have influenced the results. It's important for patients to discuss these findings with their healthcare providers, as the choice of pain management should consider individual circumstances, including potential side effects and interactions with other medications. Overall, loratadine may be a preferable option due to its better tolerability and effectiveness in reducing bone pain.
