Suicidality and Antidepressant Drugs:

Antidepressants increased the risk compared to placebo of suicidal thinking and behavior (suicidality) in children, adolescents, and young adults in short-term studies of major depressive disorder (MDD) and other psychiatric disorders. Anyone considering the use of amitriptyline hydrochloride tablets or any other antidepressant in a child, adolescent, or young adult must balance this risk with the clinical need. Short-term studies did not show an increase in the risk of suicidality with antidepressants compared to placebo in adults beyond age 24; there was a reduction in risk with antidepressants compared to placebo in adults aged 65 and older. Depression and certain other psychiatric disorders are themselves associated with increases in the risk of suicide. Patients of all ages who are started on antidepressant therapy should be monitored appropriately and observed closely for clinical worsening, suicidality, or unusual changes in behavior. Families and caregivers should be advised of the need for close observation and communication with the prescriber. Amitriptyline hydrochloride is not approved for use in pediatric patients. Overview

Amitriptyline hydrochloride tablets are a prescription medicine used for the relief of symptoms of depression. The label notes that endogenous depression is more likely to be helped than other depressive states.

This medicine is an antidepressant with sedative effects. It is not approved for use in pediatric patients, and it is not recommended for patients under 12 years of age.

Storage Information

Protect the patient's airway, establish an intravenous line and initiate gastric decontamination. Store at 20º to 25ºC (68º to 77ºF); excursions permitted to 15° to 30°C (59° to 86°F) [See USP Controlled Room Temperature].

  • Watch for worsening depression, suicidal thoughts, unusual behavior changes, agitation, irritability, insomnia, or mood changes, especially early in treatment or after dose changes.
  • Tell the prescriber right away if symptoms of serotonin syndrome occur, such as agitation, hallucinations, fast heartbeat, fever, tremor, stiffness, or diarrhea.
  • Do not use with monoamine oxidase inhibitors (MAOIs); the label lists this as a contraindication.
  • Tell the prescriber about all other medicines, including SSRIs, SNRIs, triptans, fentanyl, lithium, tramadol, buspirone, St. Johnu2019s Wort, cimetidine, and alcohol.
  • Use caution when driving or operating machinery because the medicine may impair mental and/or physical abilities.
  • Report signs of low sodium such as headache, confusion, weakness, memory problems, or unsteadiness.
  • Use caution if you have glaucoma, urinary retention, seizures, heart disease, or liver problems, as noted in the label.
  • Keep the medicine out of reach of potentially suicidal patients; the label advises prescribing the smallest feasible quantity.
