  • (5.3, 5.4) 08/2023 INDICATIONS AND USAGE Citalopram tablets are a selective serotonin reuptake inhibitor (SSRI) indicated for the treatment of major depressive disorder (MDD) in adults (1). DOSAGE AND ADMINISTRATION
  • Administer once daily with or without food (2).
  • Initial dosage is 20 mg once daily; after one week may increase to maximum dosage of 40 mg once daily (2.1).
  • Patients greater than 60 years of age, patients with hepatic impairment, and CYP2C19 poor metabolizers: maximum recommended dosage is 20 mg once daily (2.2).
  • When discontinuing citalopram tablets, reduce dosage gradually (2.4, 5.6). DOSAGE FORMS AND STRENGTHS Tablets: 10 mg; 20 mg, scored; and 40 mg, scored (3) CONTRAINDICATIONS
  • Concomitant use of monoamine oxidase inhibitors (MAOIs) or use within 14 days of discontinuing a MAOI (4).
  • Concomitant use of pimozide (4).
  • Known hypersensitivity to citalopram or any of the inactive ingredients of citalopram tablets (4). WARNINGS AND PRECAUTIONS

Adverse Reactions

Most common adverse reaction (incidence ≥ 5% and twice placebo) is ejaculation disorder (primarily ejaculation delay) (6.1).

Drug Interactions

CYP2C19 Inhibitors: Citalopram tablets 20 mg daily is the maximum recommended dosage for patients taking concomitant CYP2C19 inhibitors (5.2, 7).

Overview

Citalopram tablets are a selective serotonin reuptake inhibitor (SSRI) for oral use. The label says they are indicated for the treatment of major depressive disorder (MDD) in adults. They are not approved for use in pediatric patients.

The tablets are taken once daily, with or without food. The initial dosage is 20 mg once daily, and the dose may be increased after at least one week to a maximum of 40 mg once daily. For patients older than 60 years, patients with hepatic impairment, and CYP2C19 poor metabolizers, the maximum recommended dosage is 20 mg once daily.

Important risks in the label include suicidal thoughts and behaviors, QT prolongation and torsade de pointes, serotonin syndrome, bleeding, mania or hypomania, seizures, hyponatremia, angle-closure glaucoma, and sexual dysfunction. The label also says to taper gradually when stopping citalopram rather than stopping abruptly.

Key Counseling Points

  • Take citalopram once daily, with or without food.
  • Do not take more than the prescribed dose; doses above 40 mg once daily are not recommended.
  • If you are over 60 years old, have liver impairment, or are a CYP2C19 poor metabolizer, the maximum recommended dose is 20 mg once daily.
  • Do not use citalopram with MAOIs, or within 14 days of stopping an MAOI; the label also says to wait at least 14 days after stopping citalopram before starting an MAOI.
  • Tell your healthcare provider about all medicines you take, especially other serotonergic drugs, aspirin, NSAIDs, antiplatelet drugs, warfarin, and medicines that can prolong the QT interval.
  • Get medical help right away for symptoms that could suggest serotonin syndrome, such as agitation, hallucinations, fast heart rate, sweating, fever, tremor, rigidity, or diarrhea.
  • Report symptoms that could suggest a heart rhythm problem, such as dizziness, palpitations, or fainting.
  • Do not stop citalopram suddenly; the label says to reduce the dose gradually whenever possible.
  • Watch for worsening depression, suicidal thoughts, unusual behavior changes, or signs of mania or hypomania, especially when starting treatment or changing the dose.
  • Contact a healthcare professional if you notice bleeding, low sodium symptoms such as confusion or weakness, or changes in sexual function.
