Increased risk of suicidal thoughts and behavior in pediatric and young adult patients taking antidepressants. Closely monitor all antidepressant-treated patients for clinical worsening and emergence of suicidal thoughts and behaviors (5.1). Escitalopram tablets are not approved for use in pediatric patients less than 7 years of age (8.4).

Escitalopram is a selective serotonin reuptake inhibitor (SSRI) indicated for the:

Contraindications

Do not use MAOIs intended to treat psychiatric disorders with escitalopram or within 14 days of stopping treatment with escitalopram. Do not use escitalopram within 14 days of stopping an MAOI intended to treat psychiatric disorders. In addition, do not start escitalopram in a patient who is being treated with linezolid or intravenous methylene blue (4)

Concomitant use of pimozide (4)

Known hypersensitivity to escitalopram or citalopram or any of the inactive ingredients (4)

Adverse Reactions

Most commonly observed adverse reactions (incidence ≥ 5% and at least twice the incidence of placebo patients) are: insomnia, ejaculation disorder (primarily ejaculatory delay), nausea, sweating increased, fatigue and somnolence, decreased libido, and anorgasmia (6.1)

Drug Interactions

Use In Specific Populations

Pregnancy: SSRI use, particularly later in pregnancy, may increase the risk for persistent pulmonary hypertension and symptoms of poor adaptation (respiratory distress, temperature instability, feeding difficulties, hypotonia, tremor, irritability) in the neonate (8.1)

Overview

Escitalopram tablets are a prescription SSRI medicine taken by mouth. The label says they are used to treat major depressive disorder in adults and pediatric patients 12 years and older, and generalized anxiety disorder in adults.

The label includes a boxed warning about an increased risk of suicidal thoughts and behaviors in children, adolescents, and young adults taking antidepressants. Patients should be watched closely for worsening symptoms or new suicidal thoughts or behaviors, especially when treatment starts or when the dose changes.

Escitalopram is taken once daily, in the morning or evening, with or without food. The label also says not to stop the medicine suddenly when possible and to reduce the dose gradually when discontinuing.

Storage Information

STORAGE AND HANDLING How Supplied Escitalopram tablets, USP 5 mg are white to off-white, round, biconvex, film coated tablets debossed with '135' on one side and '5' on other side. Storage and Handling Store at 20° to 25°C (68° to 77°F); excursions permitted to 15° to 30°C (59° to 86°F) [see USP Controlled Room Temperature]. store escitalopram tablets? • Store escitalopram tablets at room temperature between 68°F to 77°F (20°C to 25°C).

  • Take escitalopram once daily, in the morning or evening, with or without food.
  • Do not change the dose or stop taking it suddenly unless directed; the label recommends gradual dose reduction when possible.
  • Watch for new or worsening mood changes, suicidal thoughts, or unusual behavior, especially early in treatment or when the dose changes.
  • Tell your prescriber about a personal or family history of bipolar disorder, mania, or hypomania before starting.
  • Avoid using escitalopram with MAOIs; the label says these combinations are contraindicated.
  • Tell your pharmacist or prescriber about all other medicines, especially serotonergic drugs, triptans, tramadol, lithium, St. Johnu2019s Wort, aspirin, NSAIDs, warfarin, or other anticoagulants.
  • Seek prompt medical attention for symptoms that could suggest serotonin syndrome, such as agitation, hallucinations, fast heartbeat, dizziness, sweating, tremor, rigidity, fever, nausea, vomiting, or diarrhea.
  • Use caution with driving or operating machinery until you know how escitalopram affects you.
  • Report unusual bleeding or bruising, since the label warns that bleeding risk can increase with certain medicines.
  • Report symptoms that may be related to low sodium, such as headache, confusion, weakness, trouble concentrating, or unsteadiness.
