  • (5.3, 5.4) 8/2023 INDICATIONS AND USAGE Citalopram is a selective serotonin reuptake inhibitor (SSRI) indicated for the treatment of major depressive disorder (MDD) in adults (1) . DOSAGE AND ADMINISTRATION
  • Administer once daily with or without food (2) .
  • Initial dosage is 20 mg once daily; after one week may increase to maximum dosage of 40 mg once daily (2.1) .
  • Patients greater than 60 years of age, patients with hepatic impairment, and CYP2C19 poor metabolizers: maximum recommended dosage is 20 mg once daily (2.2) .
  • When discontinuing citalopram tablets, reduce dosage gradually (2.4, 5.6) . DOSAGE FORMS AND STRENGTHS Tablets: 10 mg; 20 mg, scored; and 40 mg, scored (3) CONTRAINDICATIONS
  • Concomitant use of monoamine oxidase inhibitors (MAOIs) or use within 14 days of discontinuing a MAOI (4) .
  • Concomitant use of pimozide (4) .

Adverse Reactions

Most common adverse reaction (incidence ≥ 5% and twice placebo) is ejaculation disorder (primarily ejaculation delay) (6.1) .

Drug Interactions

CYP2C19 Inhibitors: Citalopram tablets 20 mg daily is the maximum recommended dosage for patients taking concomitant CYP2C19 inhibitors (5.2, 7.1).

Use In Specific Populations

Pregnancy: SSRI use, particularly late in pregnancy, may increase the risk for persistent pulmonary hypertension and symptoms of poor adaptation (respiratory distress, temperature instability, feeding difficulties, hypotonia, tremor, irritability) in the neonate (8.1).

Overview

Citalopram tablets are a selective serotonin reuptake inhibitor (SSRI) used to treat major depressive disorder in adults. The tablets are taken by mouth once daily, with or without food.

The label includes a boxed warning about suicidal thoughts and behaviors in pediatric and young adult patients taking antidepressants. Citalopram tablets are not approved for use in pediatric patients.

Important safety issues listed on the label include QT prolongation, serotonin syndrome, bleeding risk, activation of mania or hypomania, discontinuation symptoms, seizures, angle-closure glaucoma, hyponatremia, and sexual dysfunction. The dose may need to be limited in certain groups, including adults over 60 years of age, people with hepatic impairment, and CYP2C19 poor metabolizers.

STORAGE AND HANDLING Citalopram Tablets, USP contain citalopram hydrobromide USP, equivalent to 10, 20 or 40 mg citalopram base. Key Counseling Points

  • Take citalopram once daily, with or without food, exactly as prescribed.
  • Do not stop taking it suddenly; the label says the dose should be reduced gradually when discontinuing.
  • Watch for worsening depression, unusual behavior changes, or suicidal thoughts and behaviors, especially early in treatment or when the dose changes.
  • Seek urgent help for symptoms that may suggest serotonin syndrome, such as agitation, hallucinations, fever, sweating, fast heartbeat, tremor, stiffness, or diarrhea.
  • Tell the prescriber about all other medicines and supplements, especially MAOIs, other serotonergic drugs, aspirin, NSAIDs, antiplatelets, warfarin, and drugs that can affect heart rhythm.
  • The label says citalopram should not be used with MAOIs and that at least 14 days should pass between stopping one and starting the other.
  • Report symptoms that could suggest abnormal heart rhythm, such as dizziness, palpitations, or fainting.
  • Contact a healthcare professional if you notice bleeding, mood changes such as mania or hypomania, or new sexual side effects.
  • Use caution if you have a seizure disorder, narrow-angle glaucoma, or a history of low sodium, as these are listed warnings on the label.
