Increased risk of suicidal thoughts and behavior in pediatric and young adult patients taking antidepressants. Closely monitor all antidepressant-treated patients for clinical worsening and emergence of suicidal thoughts and behaviors (5.1). Escitalopram tablets are not approved for use in pediatric patients less than 7 years of age (8.4).

Warnings And Precautions

Serotonin Syndrome: Increased risk when co-administered with other serotonergic agents but also when taken alone. If it occurs, discontinue escitalopram tablets and serotonergic agents and initiate supportive treatment (4, 5.2)
Discontinuation syndrome: When discontinuing escitalopram tablets, reduce dosage gradually whenever possible, and monitor for discontinuation symptoms (5.3)
Seizures: Use with caution in patients with a history of seizure (5.4)
Activation of Mania/Hypomania: Screen patients for bipolar disorder (5.5)
Hyponatremia: Can occur in association with syndrome of inappropriate antidiuretic hormone secretion (5.6)
Increased Risk of Bleeding: Concomitant use of nonsteroidal anti-inflammatory drugs, aspirin, other antiplatelet drugs, warfarin and other drugs that affect coagulation may increase risk (5.7)
Interference with Cognitive and Motor Performance: Use caution when operating machinery (5.8)
Angle Closure Glaucoma: Angle closure glaucoma has occurred in patients with untreated anatomically narrow angles treated with antidepressants (5.9)

Adverse Reactions

Most commonly observed adverse reactions (incidence ≥ 5% and at least twice the incidence of placebo patients) are: insomnia, ejaculation disorder (primarily ejaculatory delay), nausea, sweating increased, fatigue and somnolence, decreased libido, and anorgasmia (6.1)

Overview

Escitalopram tablets are an SSRI medicine taken by mouth. The label says they are used to treat major depressive disorder in adults and in pediatric patients 12 years and older, and generalized anxiety disorder in adults and in pediatric patients 7 years and older.

The label includes a boxed warning about an increased risk of suicidal thoughts and behaviors in pediatric and young adult patients taking antidepressants. Patients should be monitored closely for clinical worsening and for new suicidal thoughts or behaviors.

Escitalopram is taken once daily, with or without food, in the morning or evening. The label also says not to stop it suddenly when possible, and to reduce the dose gradually when discontinuing.

The tablets are available in 5 mg, 10 mg, and 20 mg strengths. Commonly observed adverse reactions include insomnia, ejaculation disorder, nausea, increased sweating, fatigue, somnolence, decreased libido, and anorgasmia.

Storage Information

STORAGE AND HANDLING How Supplied Escitalopram Tablets USP, 5 mg are round, white to off-white, biconvex, film coated tablets, debossed with “P 5” on one side and plain on the other side. Storage and Handling Store at 20°C to 25°C (68°F to 77°F); excursions permitted to 15°C to 30°C (59°F to 86°F) [See USP Controlled Room Temperature]. store escitalopram tablets? •Store escitalopram tablets at room temperature between 68°F to 77°F (20°C to 25°C). Key Counseling Points

Take escitalopram once daily, in the morning or evening, with or without food.
Do not stop taking escitalopram suddenly unless instructed; the label recommends gradual dose reduction when possible.
Tell your healthcare provider about any personal or family history of bipolar disorder, mania, or hypomania before starting.
Watch for new or worsening suicidal thoughts or behavior, especially early in treatment and when the dose changes.
Get medical help right away if symptoms of serotonin syndrome occur, such as agitation, hallucinations, fever, sweating, shaking, stiff muscles, or diarrhea.
Tell your healthcare provider about all medicines you take, especially MAOIs, other antidepressants, triptans, tramadol, lithium, or St. Johnu2019s wort.
Use caution with driving or operating machinery until you know how escitalopram affects you.
Tell your healthcare provider if you have a history of seizures, bleeding problems, low sodium, or eye problems such as narrow angles.
Sexual side effects can occur, including decreased libido, delayed orgasm, or erectile dysfunction.
Common side effects reported in the label include insomnia, nausea, sweating, fatigue, somnolence, and decreased libido.
