Suicidality and Antidepressant Drugs:
Antidepressants increased the risk compared to placebo of suicidal thinking and behavior (suicidality) in children, adolescents, and young adults in short-term studies of major depressive disorder (MDD) and other psychiatric disorders. Anyone considering the use of amitriptyline hydrochloride tablets or any other antidepressant in a child, adolescent, or young adult must balance this risk with the clinical need. Short-term studies did not show an increase in the risk of suicidality with antidepressants compared to placebo in adults beyond age 24; there was a reduction in risk with antidepressants compared to placebo in adults aged 65 and older. Depression and certain other psychiatric disorders are themselves associated with increases in the risk of suicide. Patients of all ages who are started on antidepressant therapy should be monitored appropriately and observed closely for clinical worsening, suicidality, or unusual changes in behavior. Families and caregivers should be advised of the need for close observation and communication with the prescriber. Amitriptyline hydrochloride is not approved for use in pediatric patients. Overview
Amitriptyline hydrochloride tablets are a tricyclic antidepressant used for the relief of symptoms of depression. The label notes that endogenous depression is more likely to be helped than other depressive states.
The medicine has important warnings. Antidepressants, including amitriptyline, may be associated with worsening depression, suicidal thoughts, or unusual changes in behavior, especially early in treatment or when the dose changes. Patients, families, and caregivers should watch closely for these changes and report them promptly.
Amitriptyline can also interact with many other medicines and may not be used with monoamine oxidase inhibitors (MAOIs). The label also warns about serotonin syndrome, heart rhythm effects, seizure risk, glaucoma, urinary retention, and increased sensitivity in older adults. It is not approved for pediatric use and is not recommended for patients under 12 years of age.
Storage Information
Protect the patient's airway, establish an intravenous line and initiate gastric decontamination. Store at 20º to 25ºC (68º to 77ºF); excursions permitted to 15° to 30°C (59° to 86°F) [See USP Controlled Room Temperature].
Key Counseling Points
- Take amitriptyline exactly as prescribed.
- Do not use it with MAO inhibitors; the label requires a 14-day gap when switching from an MAOI.
- Tell your prescriber about all other medicines, including SSRIs/SNRIs, triptans, tramadol, lithium, buspirone, St. Johnu2019s Wort, linezolid, and intravenous methylene blue.
- Watch for new or worsening depression, suicidal thoughts, agitation, irritability, unusual behavior changes, or mania, especially early in treatment or after dose changes.
- Get urgent help if symptoms of serotonin syndrome occur, such as agitation, hallucinations, fever, sweating, fast heartbeat, tremor, stiffness, or diarrhea.
- Use caution with alcohol and other CNS depressants because amitriptyline can increase their effects.
- Be aware that the medicine may cause drowsiness or affect the ability to drive or use machinery.
- Tell a healthcare professional if you have glaucoma, urinary retention, seizures, heart disease, liver problems, or a history of bipolar disorder.
- Older adults may be more sensitive to side effects such as confusion, dry mouth, constipation, blurred vision, urinary retention, and falls.
- If symptoms of hyponatremia occur, such as headache, confusion, weakness, or unsteadiness, seek medical attention promptly.
