- (5.3, 5.4) 08/2023 INDICATIONS AND USAGE Citalopram tablets are a selective serotonin reuptake inhibitor (SSRI) indicated for the treatment of major depressive disorder (MDD) in adults (1). DOSAGE AND ADMINISTRATION
- Administer once daily with or without food (2).
- Initial dosage is 20 mg once daily; after one week may increase to maximum dosage of 40 mg once daily (2.1).
- Patients greater than 60 years of age, patients with hepatic impairment, and CYP2C19 poor metabolizers: maximum recommended dosage is 20 mg once daily (2.2).
- When discontinuing citalopram tablets, reduce dosage gradually (2.4, 5.6). DOSAGE FORMS AND STRENGTHS Tablets: 10 mg; 20 mg, scored; and 40 mg, scored (3) CONTRAINDICATIONS
- Concomitant use of monoamine oxidase inhibitors (MAOIs) or use within 14 days of discontinuing a MAOI (4).
- Concomitant use of pimozide (4).

Adverse Reactions
Most common adverse reaction (incidence ≥ 5% and twice placebo) is ejaculation disorder (primarily ejaculation delay) (6.1).

Drug Interactions
CYP2C19 Inhibitors: Citalopram tablets 20 mg daily is the maximum recommended dosage for patients taking concomitant CYP2C19 inhibitors (5.2, 7).

Overview
Citalopram tablets are a selective serotonin reuptake inhibitor (SSRI) for oral use. According to the label, they are indicated for the treatment of major depressive disorder (MDD) in adults.
The tablets are taken once daily, with or without food. The usual starting dose is 20 mg once daily, and after at least one week it may be increased to a maximum of 40 mg once daily. For people older than 60 years, people with hepatic impairment, and CYP2C19 poor metabolizers, the maximum recommended dose is 20 mg once daily.
The label includes a boxed warning about suicidal thoughts and behaviors in pediatric and young adult patients taking antidepressants. Citalopram tablets are not approved for use in pediatric patients. The label also warns about risks including QT prolongation, serotonin syndrome, bleeding, mania or hypomania, seizures, angle-closure glaucoma, hyponatremia, and sexual dysfunction.
When stopping treatment, the dose should be reduced gradually rather than stopped abruptly whenever possible. The label says adverse reactions may occur when citalopram is discontinued.
Key Counseling Points
- Take once daily, with or without food, exactly as prescribed.
- Do not take more than 40 mg once daily; some patients should not take more than 20 mg once daily.
- Do not use with MAO inhibitors, and allow at least 14 days between citalopram and an MAOI.
- Tell the healthcare provider about all medicines, including drugs that may prolong the QT interval or increase serotonin syndrome risk.
- Watch for suicidal thoughts, worsening depression, or unusual behavior changes, especially early in treatment or when the dose changes.
- Seek help right away for symptoms of serotonin syndrome such as agitation, hallucinations, fast heartbeat, fever, stiffness, tremor, nausea, vomiting, or diarrhea.
- Report symptoms that may suggest heart rhythm problems, such as dizziness, palpitations, or fainting.
- Do not stop citalopram suddenly; the label recommends gradually lowering the dose when possible.
- Use caution about bleeding risk if taking aspirin, NSAIDs, antiplatelet drugs, warfarin, or other anticoagulants.
- The label notes possible sexual side effects, including decreased libido, delayed ejaculation, erectile dysfunction, or trouble having an orgasm.
