Indications And Usage

Clopidogrel is a P2Y

12platelet inhibitor indicated for:

Acute coronary syndrome

For patients with non–ST-segment elevation ACS (unstable angina [UA]/non-ST-elevation myocardial infarction [NSTEMI]), clopidogrel has been shown to reduce the rate of myocardial infarction (MI) and stroke. (

Initiate clopidogrel with a single 300-mg oral loading dose and then continue at 75 mg once daily.

Initiate clopidogrel without a loading dose will delay establishment of an antiplatelet effect by several days.

Drug Interactions

CYP2C19 inducers: Increases levels of clopidogrel active metabolite and increases platelet inhibition. (

Opioids: Decreased exposure to clopidogrel. Consider use of parenteral antiplatelet agent.

Nonsteroidal anti-inflammatory drugs (NSAIDs), warfarin, selective serotonin and serotonin norepinephrine reuptake inhibitors (SSRIs, SNRIs): Increases risk of bleeding.

Overview

Clopidogrel tablets are a P2Y12 platelet inhibitor used to help reduce the rate of heart attack and stroke in certain patients with acute coronary syndrome, recent heart attack, recent stroke, or established peripheral arterial disease. For some acute coronary syndrome patients, the tablet is given with aspirin.

The label says clopidogrel works by blocking platelet activation and aggregation after it is converted to an active metabolite in the body. It also warns that the medicine may be less effective in people with two loss-of-function CYP2C19 alleles, and that tests are available to identify CYP2C19 poor metabolizers.

Bleeding is the most commonly reported adverse reaction. The label also warns about serious bleeding, thrombotic thrombocytopenic purpura (TTP), and reduced effect when used with omeprazole or esomeprazole. Stopping clopidogrel too soon can raise the risk of cardiovascular events.

Storage Information

store hemostasis by administering exogenous platelets; however, platelet transfusions within 4 hours of the loading dose or 2 hours of the maintenance dose may be less effective. STORAGE AND HANDLING Clopidogrel tablets, USP 75 mg are available as pink colored, round shaped, biconvex, film coated tablets de-bossed on one side with SG and 124 on other side. Store at 25° C (77° F); excursions permitted to 15° to 30° C (59° to 86° F) [see USP Controlled Room Temperature]. store clopidogrel tablets?   • Take clopidogrel exactly as prescribed.
  • Clopidogrel may be taken with or without food.
  • Tell your healthcare provider about all medicines you take, including omeprazole or esomeprazole.
  • Watch for signs of bleeding, since clopidogrel can increase bleeding risk.
  • Do not stop clopidogrel without medical direction, because stopping can increase the risk of cardiovascular events.
  • Let your healthcare provider know if you have a history of bleeding or an active bleeding condition.
  • Tell your healthcare provider if you have had a reaction to clopidogrel or other thienopyridines.
  • Seek urgent attention for symptoms listed in the label for TTP, such as low platelets, anemia, neurologic findings, kidney problems, or fever.
  • Tell your healthcare provider if you are scheduled for surgery, because the label says clopidogrel may need to be interrupted 5 days before surgery with a major risk of bleeding.
