Suicidality and Antidepressant Drugs
Antidepressants increased the risk compared to placebo of suicidal thinking and behavior (suicidality) in children, adolescents and young adults in short-term studies of major depressive disorder (MDD) and other psychiatric disorders. Anyone considering the use of doxepin or any other antidepressant in a child, adolescent, or young adult must balance this risk with the clinical need. Short-term studies did not show an increase in the risk of suicidality with antidepressants compared to placebo in adults beyond age 24; there was a reduction in risk with antidepressants compared to placebo in adults aged 65 and older. Depression and certain other psychiatric disorders are themselves associated with increases in the risk of suicide. Patients of all ages who are started on antidepressant therapy should be monitored appropriately and observed closely for clinical worsening, suicidality, or unusual changes in behavior. Families and caregivers should be advised of the need for close observation and communication with the prescriber. Doxepin is not approved for use in pediatric patients. Overview
Doxepin hydrochloride capsules are a tricyclic psychotherapeutic medicine used to treat depression and anxiety in certain patients. The label lists use for psychoneurotic patients with depression and/or anxiety, depression and/or anxiety associated with alcoholism or organic disease, and psychotic depressive disorders with associated anxiety.
The capsules come in multiple strengths, including 10 mg, 25 mg, 50 mg, 75 mg, and 100 mg. The label says doxepin is not recommended for children under 12 years of age, and it is not approved for pediatric use.
The label warns that antidepressants may increase suicidal thoughts or behavior in children, adolescents, and young adults, so patients should be monitored closely for worsening depression, unusual behavior changes, or suicidality. Drowsiness, dry mouth, blurred vision, constipation, and urinary retention are among the reported adverse effects.
The label also notes important cautions about glaucoma, urinary retention, alcohol, and interactions with other medicines, including MAO inhibitors and some drugs that affect CYP2D6.
Storage Information
Protect the patient’s airway, establish an intravenous line and initiate gastric decontamination. Store at 20°C to 25°C (68°F to 77°F). Dispense in a tight, light-resistant container as defined in the USP using a child-resistant closure.
