Overview
Enalapril maleate tablets are a prescription medicine used to treat high blood pressure. They are also used to treat symptomatic congestive heart failure and, in clinically stable patients with left ventricular dysfunction, to help decrease the development of overt heart failure and reduce hospitalizations for heart failure.
Enalapril is an ACE inhibitor. After it is absorbed, it is changed in the body to enalaprilat, the active form. The label says blood pressure effects can begin within about 1 hour after a dose, with peak effect in 4 to 6 hours, and the effect can last at least 24 hours in many patients.
The label includes a boxed warning about fetal toxicity. If pregnancy is detected, enalapril maleate tablets should be stopped as soon as possible. The medicine should not be used in certain patients with a history of angioedema or with some other medicines, as described in the label.

Key Counseling Points
- This medicine is taken by mouth as directed on the prescription label.
- Do not use it if you have a history of angioedema related to an ACE inhibitor or hereditary/idiopathic angioedema.
- Tell a healthcare professional right away if you notice swelling of the face, lips, tongue, throat, or trouble breathing or swallowing.
- Report lightheadedness or fainting, especially when starting treatment.
- Dehydration, vomiting, diarrhea, or heavy sweating can increase the chance of low blood pressure.
- Do not use potassium-containing salt substitutes without first discussing it with a healthcare professional.
- Report persistent cough, which can occur with ACE inhibitors.
- If you have kidney disease, diabetes, or are taking potassium-sparing diuretics or potassium supplements, the label says extra caution is needed.
- This medicine should be stopped as soon as possible if pregnancy is detected, according to the boxed warning.
- Do not take enalapril within 36 hours of switching to or from sacubitril/valsartan.
