METOPROLOL SUCCINATE extended-release tablets, for oral use

Initial U.S. Approval: 1992

Indications And Usage

Metoprolol succinate extended-release tablets, are a beta-adrenergic blocker indicated for the treatment of:

Hypertension, to lower blood pressure. Lowering blood pressure reduces the risk of fatal and non-fatal cardiovascular events, primarily strokes and myocardial infarctions. (

Angina Pectoris. (

Heart Failure, to reduce the risk of cardiovascular mortality and heart failure hospitalizations in patients with heart failure (

Dosage And Administration

Administer once daily. Titrate at weekly or longer intervals as needed and tolerated. (

Hypertension: Starting dose is 25 to 100 mg. (

Angina Pectoris: Starting dose is 100 mg. (

2.2)

Heart Failure: Starting dose is 12.5 or 25 mg. (

2.3)

Switching from immediate-release metoprolol to metoprolol succinate extended-release tablets: use the same total daily dose of metoprolol succinate extended-release tablets. (

Dosage Forms And Strengths

Metoprolol succinate extended-release tablets: 25 mg, 50 mg, 100 mg, and 200 mg. (

Contraindications

Known hypersensitivity to product components. (

4)

Severe bradycardia: Greater than first degree heart block, or sick sinus syndrome without a pacemaker. (

Cardiogenic shock or decompensated heart failure. (

Warnings And Precautions

  • Abrupt cessation may exacerbate myocardial ischemia. ( 5.1)
  • Heart Failure: Worsening cardiac failure may occur. ( 5.2)
  • Bronchospastic Disease: Avoid beta-blockers. ( 5.3)
  • Concomitant use of glycosides, clonidine, diltiazem and verapamil with beta-blockers can increase the risk of bradycardia. ( 5.4)
  • Pheochromocytoma: Initiate therapy with an alpha-blocker. ( 5.5)
  • Major Surgery: Avoid initiation of high-dose extended-release metoprolol in patients undergoing non-cardiac surgery. Do not routinely withdraw chronic beta blocker therapy prior to surgery. ( 5.6, 6.1)
  • Hypoglycemia: May increase risk for hypoglycemia and mask early warning signs. ( 5.7)
  • Thyrotoxicosis: Abrupt withdrawal in patients with thyrotoxicosis might precipitate a thyroid storm. ( 5.8)

Patients may be unresponsive to the usual doses of epinephrine used to treat allergic reaction. (

Most common adverse reactions: tiredness, dizziness, depression, shortness of breath, bradycardia, hypotension, diarrhea, pruritus, rash. (

Drug Interactions

Catecholamine-depleting drugs may have an additive effect when given with beta-blocking agents. (

CYP2D6 Inhibitors are likely to increase metoprolol concentration. (

Beta-blockers including metoprolol, may exacerbate the rebound hypertension that can follow the withdrawal of clonidine. (

Use In Specific Populations

Hepatic Impairment: Consider initiating metoprolol succinate extended-release tablets therapy at low doses and gradually increase dosage to optimize therapy, while monitoring closely for adverse events. (

Overview

Metoprolol succinate extended-release tablets are a beta-blocker taken by mouth once daily. They are used to treat high blood pressure, angina (chest pain), and heart failure.

For high blood pressure, the medicine is used to lower blood pressure and may help reduce the risk of stroke and heart attack. For angina, it is used long term to reduce angina attacks and improve exercise tolerance.

For heart failure, metoprolol succinate extended-release tablets are used to reduce the risk of cardiovascular death and hospitalization for heart failure. The dose is adjusted over time based on response and tolerance, and the tablet should not be crushed or chewed.

Storage Information

store clinical stability before advancing the dose of metoprolol succinate extended-release tablets [see Dosage and Administration (2)] . STORAGE AND HANDLING Metoprolol succinate extended-release tablets, USP containing metoprolol succinate USP equivalent to the indicated weight of metoprolol tartrate, USP, are available as follows: 25 mg:White to off white, oval, biconvex film coated tablets score line on both sides debossed with 'J' on one side and '75' on the other side separating 7 & 5 with score line. Store at 20° to 25°C (68° to 77°F) [see USP Controlled Room Temperature].

  • Take metoprolol succinate extended-release tablets once daily as directed.
  • Do not crush or chew the whole tablet or half tablet.
  • The tablet is scored and can be divided if needed.
  • Do not stop the medicine suddenly unless told to do so; the label says stopping abruptly can worsen chest pain and may increase the risk of heart attack.
  • Tell a healthcare professional if you notice a very slow heart rate, dizziness, tiredness, shortness of breath, or fainting.
  • The medicine may worsen heart failure when the dose is being increased; dose changes are done slowly and monitored closely.
  • Use caution if you have bronchospastic disease, since beta-blockers are generally avoided and the lowest possible dose may be used if prescribed.
  • This medicine can mask early warning signs of low blood sugar and may increase the risk of severe or prolonged hypoglycemia.
  • Tell a healthcare professional about other medicines you take, especially clonidine, diltiazem, verapamil, digoxin, MAO inhibitors, or strong CYP2D6 inhibitors such as quinidine, fluoxetine, paroxetine, and propafenone.
  • Seek urgent help for serious allergic reactions, severe breathing problems, or symptoms of very low blood pressure or severe bradycardia.
