Suicidality and Antidepressant Drugs

Antidepressants increased the risk compared to placebo of suicidal thinking and behavior (suicidality) in children, adolescents, and young adults in short-term studies of Major Depressive Disorder (MDD) and other psychiatric disorders. Anyone considering the use of venlafaxine tablets or any other antidepressant in a child, adolescent, or young adult must balance this risk with the clinical need. Short-term studies did not show an increase in the risk of suicidality with antidepressants compared to placebo in adults beyond age 24; there was a reduction in risk with antidepressants compared to placebo in adults aged 65 and older. Depression and certain other psychiatric disorders are themselves associated with increases in the risk of suicide. Patients of all ages who are started on antidepressant therapy should be monitored appropriately and observed closely for clinical worsening, suicidality, or unusual changes in behavior. Families and caregivers should be advised of the need for close observation and communication with the prescriber. Venlafaxine tablets are not approved for use in pediatric patients (see WARNINGS: Clinical Worsening and Suicide Risk, PRECAUTIONS: Information for Patients, and PRECAUTIONS: Pediatric Use).

Overview

Venlafaxine tablets, USP are an oral antidepressant used to treat major depressive disorder. The label describes venlafaxine as a structurally novel antidepressant and notes that its active metabolite is O-desmethylvenlafaxine (ODV).

The medicine is not approved for use in pediatric patients. The label also says patients should be monitored for worsening depression, suicidal thoughts or behavior, and unusual changes in behavior, especially when treatment starts or the dose changes.

Venlafaxine should not be used with MAOIs intended to treat psychiatric disorders, or within certain time windows around MAOI use, because of the risk of serotonin syndrome. The label also warns about sustained increases in blood pressure, angle-closure glaucoma, discontinuation symptoms, and possible sexual side effects.

  • Take venlafaxine exactly as prescribed.
  • Do not stop taking it suddenly unless your prescriber tells you to; the label says abrupt stopping can cause withdrawal symptoms.
  • Watch for worsening depression, suicidal thoughts, unusual behavior changes, agitation, irritability, or insomnia, especially early in treatment or when the dose changes.
  • Tell your prescriber about all medicines and supplements you take, especially MAOIs and other serotonergic drugs, because of serotonin syndrome risk.
  • Know the symptoms of serotonin syndrome, which can include agitation, hallucinations, fast heart rate, blood pressure changes, tremor, rigidity, fever, nausea, vomiting, or diarrhea.
  • Blood pressure should be monitored during treatment because venlafaxine can cause sustained increases in blood pressure.
  • Report eye pain or vision changes right away, since the label warns about angle-closure glaucoma.
  • Ask about sexual side effects if they occur, because decreased libido, delayed orgasm, erectile dysfunction, or ejaculatory delay can happen.
  • If you miss a dose or have side effects, contact your prescriber or pharmacist for guidance.
  • The label says this medicine is not approved for pediatric patients.
