Vardenafil safety begins before the first dose. Because the medication can affect blood vessels and blood pressure, patients should only use it when prescribed and after a healthcare provider reviews their health history and medication list.
The most important rule is simple: vardenafil must not be combined with nitrate medications or nitric oxide donors. This combination can cause a dangerous drop in blood pressure.
Nitrates and chest pain medications
Nitrates may be prescribed for chest pain or angina. Some recreational products known as “poppers” can also act like nitrates. Patients should be honest with their provider about all substances they use, even if they feel embarrassed. The goal is safety, not judgment.
If chest pain occurs after taking vardenafil, patients should seek emergency care and tell the medical team that they used an ED medication.
Alpha-blockers and blood pressure medications
Alpha-blockers are sometimes used for prostate symptoms or blood pressure. When combined with PDE5 inhibitors, they can contribute to low blood pressure in some patients. That does not mean every patient is automatically excluded, but it does mean the prescriber needs to evaluate risk carefully.
Patients who take multiple blood pressure medicines, have fainting episodes, or have unstable heart symptoms should ask for specific guidance before using vardenafil.
Heart health and ED
ED can sometimes be an early sign of blood vessel problems. Men with new or worsening ED should consider discussing cardiovascular risk factors such as blood pressure, cholesterol, diabetes, smoking, weight, and exercise with their healthcare provider.
Vardenafil may help with erections, but it does not replace appropriate evaluation of underlying health concerns.

Medication Safety Note
Vardenafil is a prescription medication and is not appropriate for everyone. Do not take vardenafil with nitrate medications or nitric oxide donors, and tell your healthcare provider about heart conditions, blood pressure medicines, alpha-blockers, liver or kidney problems, vision or hearing history, and every medication or supplement you use. Seek urgent medical help for chest pain, fainting, sudden vision or hearing changes, or an erection lasting longer than 4 hours.
