Journal Article Summary
The article investigates the effectiveness of two treatment approaches for Helicobacter pylori (H. pylori) infection in patients with gastric neoplasms or gastric mucosa-associated lymphoid tissue lymphoma. H. pylori is a significant cause of various gastrointestinal diseases, including gastritis and gastric cancer, making its effective eradication crucial. The study compares empirical therapy, which uses a standard treatment regimen, to tailored therapy guided by clarithromycin resistance testing, as antibiotic resistance is a growing concern that can lead to treatment failure.
In this randomized controlled trial, 72 patients were enrolled and divided into two groups: one receiving empirical therapy and the other receiving tailored therapy based on genetic testing for clarithromycin resistance. The results showed that the eradication rates were 75% for the empirical group and 88.9% for the tailored group in the intention-to-treat analysis. However, in a per-protocol analysis, the tailored therapy group had a significantly higher eradication rate of 97% compared to 81.8% for the empirical group. Notably, tailored therapy was more effective in eradicating H. pylori in patients with clarithromycin resistance.
Despite these promising findings, the study has limitations, including being conducted at a single center, which may affect the generalizability of the results. Additionally, while tailored therapy showed higher efficacy, it also resulted in more adverse effects, although these were generally mild and did not impact patient compliance. Patients should discuss their treatment options with healthcare professionals, especially regarding the potential for antibiotic resistance and the best approach for their specific condition.
