Suicidality and Antidepressant Drugs:
Antidepressants increased the risk compared to placebo of suicidal thinking and behavior (suicidality) in children, adolescents, and young adults in short-term studies of major depressive disorder (MDD) and other psychiatric disorders. Anyone considering the use of amitriptyline hydrochloride tablets or any other antidepressant in a child, adolescent, or young adult must balance this risk with the clinical need. Short-term studies did not show an increase in the risk of suicidality with antidepressants compared to placebo in adults beyond age 24; there was a reduction in risk with antidepressants compared to placebo in adults aged 65 and older. Depression and certain other psychiatric disorders are themselves associated with increases in the risk of suicide. Patients of all ages who are started on antidepressant therapy should be monitored appropriately and observed closely for clinical worsening, suicidality, or unusual changes in behavior. Families and caregivers should be advised of the need for close observation and communication with the prescriber. Amitriptyline hydrochloride is not approved for use in pediatric patients. Overview
Amitriptyline hydrochloride tablets are a prescription antidepressant used for the relief of symptoms of depression. The label notes that endogenous depression may be more likely to improve than other types of depressive states.
This medicine has sedative effects and is not a monoamine oxidase inhibitor. It should not be used with monoamine oxidase inhibitors, and it should not be used with cisapride. It is also not recommended during the acute recovery phase after a heart attack.
The label warns that antidepressants may increase the risk of suicidal thinking and behavior in children, adolescents, and young adults, especially early in treatment or when the dose is changed. Patients, families, and caregivers should watch closely for worsening depression, unusual behavior changes, or suicidal thoughts and report them promptly.
Amitriptyline hydrochloride has important warnings about serotonin syndrome, heart rhythm effects, seizures, glaucoma, urinary retention, low sodium, and interactions with alcohol and many other medicines.
Storage Information
Protect the patient's airway, establish an intravenous line and initiate gastric decontamination. Store at 20º to 25ºC (68º to 77ºF); excursions permitted to 15° to 30°C (59° to 86°F) [See USP Controlled Room Temperature].
Key Counseling Points
- Take this medicine exactly as prescribed.
- Do not use it with monoamine oxidase inhibitors.
- Do not start it if you are being treated with linezolid or intravenous methylene blue.
- Report worsening depression, unusual behavior changes, or suicidal thoughts right away.
- Watch for symptoms of serotonin syndrome such as agitation, hallucinations, fever, sweating, tremor, stiff muscles, or diarrhea.
- Use caution with alcohol and other central nervous system depressants because the medicine may enhance their effects.
- Tell the prescriber about a history of seizures, urinary retention, glaucoma, heart disease, thyroid treatment, or liver problems.
- This medicine may cause sleepiness or impair thinking and physical abilities needed for driving or operating machinery.
- Older adults may be more sensitive to anticholinergic effects such as dry mouth, constipation, blurred vision, confusion, and urinary retention.
- If symptoms of low sodium occur, such as headache, confusion, weakness, or unsteadiness, seek medical attention promptly.
