Suicidality and Antidepressant Drugs:
Antidepressants increased the risk compared to placebo of suicidal thinking and behavior (suicidality) in children, adolescents, and young adults in short-term studies of major depressive disorder (MDD) and other psychiatric disorders. Anyone considering the use of amitriptyline hydrochloride tablets or any other antidepressant in a child, adolescent, or young adult must balance this risk with the clinical need. Short-term studies did not show an increase in the risk of suicidality with antidepressants compared to placebo in adults beyond age 24; there was a reduction in risk with antidepressants compared to placebo in adults aged 65 and older. Depression and certain other psychiatric disorders are themselves associated with increases in the risk of suicide. Patients of all ages who are started on antidepressant therapy should be monitored appropriately and observed closely for clinical worsening, suicidality, or unusual changes in behavior. Families and caregivers should be advised of the need for close observation and communication with the prescriber. Amitriptyline hydrochloride is not approved for use in pediatric patients. Overview
Amitriptyline hydrochloride tablets are a prescription medicine used for the relief of symptoms of depression. The label notes that endogenous depression is more likely to be helped than other depressive states.
This medicine is a tricyclic antidepressant. It has sedative effects, and its exact mechanism of action in people is not known.
The label includes important warnings about suicidality risk, especially in children, adolescents, and young adults, and says patients should be monitored closely for worsening depression, unusual behavior changes, or suicidal thoughts. It also warns about serotonin syndrome, interactions with other medicines, and other serious safety concerns.

Storage Information
Protect the patient's airway, establish an intravenous line and initiate gastric decontamination. Store at 20º to 25ºC (68º to 77ºF); excursions permitted to 15° to 30°C (59° to 86°F) [See USP Controlled Room Temperature].

- Take this medicine only as prescribed.
- Read the Medication Guide and ask the prescriber or pharmacist about any questions.
- Watch for worsening depression, new suicidal thoughts, or unusual changes in behavior, especially early in treatment or when the dose changes.
- Tell the prescriber right away about symptoms that may signal serotonin syndrome, such as agitation, confusion, hallucinations, fever, sweating, tremor, rigidity, or diarrhea.
- Do not use with monoamine oxidase inhibitors; the label says some combinations are contraindicated.
- Tell the prescriber about all other medicines you take, including SSRIs, SNRIs, triptans, fentanyl, lithium, tramadol, buspirone, tryptophan, St. John's Wort, linezolid, and intravenous methylene blue.
- Use caution with alcohol, barbiturates, and other CNS depressants, because the medicine may enhance their effects.
- Be aware that the medicine may cause drowsiness or impair mental and physical abilities needed for driving or operating machinery.
- Report symptoms of hyponatremia such as headache, confusion, weakness, unsteadiness, or difficulty concentrating.
- Report eye pain or vision changes, especially if you have narrow angles or glaucoma, since the label warns about angle-closure glaucoma.
