Suicidality and Antidepressant Drugs:
Antidepressants increased the risk compared to placebo of suicidal thinking and behavior (suicidality) in children, adolescents, and young adults in short-term studies of major depressive disorder (MDD) and other psychiatric disorders. Anyone considering the use of amitriptyline hydrochloride tablets or any other antidepressant in a child, adolescent, or young adult must balance this risk with the clinical need. Short-term studies did not show an increase in the risk of suicidality with antidepressants compared to placebo in adults beyond age 24; there was a reduction in risk with antidepressants compared to placebo in adults aged 65 and older. Depression and certain other psychiatric disorders are themselves associated with increases in the risk of suicide. Patients of all ages who are started on antidepressant therapy should be monitored appropriately and observed closely for clinical worsening, suicidality, or unusual changes in behavior. Families and caregivers should be advised of the need for close observation and communication with the prescriber. Amitriptyline hydrochloride is not approved for use in pediatric patients. Overview
Amitriptyline hydrochloride tablets are a prescription antidepressant used for the relief of symptoms of depression. The label notes that endogenous depression is more likely to be helped than other depressive states.
This medicine has sedative effects and is taken by mouth. It is not a monoamine oxidase inhibitor.
The label includes important warnings about worsening depression, suicidal thoughts or behavior, serotonin syndrome, and other serious side effects. Patients should be monitored closely, especially when treatment starts or the dose changes.
Amitriptyline hydrochloride is not approved for use in pediatric patients, and it is not recommended for patients under 12 years of age.
Storage Information
Protect the patient's airway, establish an intravenous line and initiate gastric decontamination. Store at 20º to 25ºC (68º to 77ºF); excursions permitted to 15° to 30°C (59° to 86°F) [See USP Controlled Room Temperature].
Key Counseling Points
- Take this medicine only as prescribed for depression.
- Watch for worsening depression, unusual behavior changes, or suicidal thoughts and contact the prescriber right away if they occur.
- Be aware that serotonin syndrome has been reported, especially with other serotonergic medicines.
- Tell the prescriber about all medicines, including MAOIs, SSRIs/SNRIs, triptans, fentanyl, lithium, tramadol, buspirone, tryptophan, St. Johnu2019s Wort, linezolid, and intravenous methylene blue.
- Do not use with monoamine oxidase inhibitors; the label requires a 14-day interval when switching from an MAOI to amitriptyline hydrochloride.
- Use caution with alcohol and other CNS depressants because the medicine may enhance their effects.
- This medicine may cause drowsiness or impair thinking and physical abilities needed for driving or operating machinery.
- Report symptoms of hyponatremia such as headache, confusion, weakness, unsteadiness, or difficulty concentrating.
- Tell the prescriber if you have a history of seizures, urinary retention, narrow-angle glaucoma, heart problems, or liver problems.
- If you are pregnant, breastfeeding, or caring for a child, note that the label includes special warnings and age-related precautions.
