Increased risk of suicidal thoughts and behavior in pediatric and young adult patients taking antidepressants. Closely monitor all antidepressant-treated patients for clinical worsening and emergence of suicidal thoughts and behaviors (5.1). Escitalopram tablets are not approved for use in pediatric patients less than 7 years of age (8.4).

Warnings And Precautions

Serotonin Syndrome: Increased risk when co-administered with other serotonergic agents but also when taken alone. If it occurs, discontinue escitalopram tablets and serotonergic agents and initiate supportive treatment (4, 5.2)
Discontinuation syndrome: When discontinuing escitalopram tablets, reduce dosage gradually whenever possible, and monitor for discontinuation symptoms (5.3)
Seizures: Use with caution in patients with a history of seizure (5.4)
Activation of Mania/Hypomania: Screen patients for bipolar disorder (5.5)
Hyponatremia: Can occur in association with syndrome of inappropriate antidiuretic hormone secretion (5.6)
Increased Risk of Bleeding: Concomitant use of nonsteroidal anti-inflammatory drugs, aspirin, other antiplatelet drugs, warfarin and other drugs that affect coagulation may increase risk (5.7)
Interference with Cognitive and Motor Performance: Use caution when operating machinery (5.8)
Angle Closure Glaucoma: Angle closure glaucoma has occurred in patients with untreated anatomically narrow angles treated with antidepressants (5.9)

Adverse Reactions

Most commonly observed adverse reactions (incidence ≥ 5% and at least twice the incidence of placebo patients) are: insomnia, ejaculation disorder (primarily ejaculatory delay), nausea, sweating increased, fatigue and somnolence, decreased libido, and anorgasmia (6.1)

Overview

Escitalopram tablets are a selective serotonin reuptake inhibitor (SSRI) taken by mouth. The label says they are used to treat major depressive disorder in adults and in pediatric patients 12 years of age and older, and generalized anxiety disorder in adults and in pediatric patients 7 years of age and older.

The label includes a boxed warning about suicidal thoughts and behaviors in children, adolescents, and young adults taking antidepressants. Patients should be monitored closely for clinical worsening and for new or worsening suicidal thoughts or behaviors.

Escitalopram is usually taken once daily, in the morning or evening, with or without food. The tablets come in 5 mg, 10 mg, and 20 mg strengths, and the 5 mg tablet is round, white to off-white, biconvex, and film coated with “P 5” on one side and plain on the other.

Storage Information

STORAGE AND HANDLING How Supplied Escitalopram Tablets USP, 5 mg are round, white to off-white, biconvex, film coated tablets, debossed with “P 5” on one side and plain on the other side. Storage and Handling Store at 20°C to 25°C (68°F to 77°F); excursions permitted to 15°C to 30°C (59°F to 86°F) [See USP Controlled Room Temperature]. store escitalopram tablets? •Store escitalopram tablets at room temperature between 68°F to 77°F (20°C to 25°C). Key Counseling Points

Take escitalopram once daily, in the morning or evening, with or without food, as directed on the label.
Tell your healthcare provider about all medicines you take, especially MAOIs, pimozide, and other serotonergic medicines.
Do not use escitalopram with an MAOI, or within 14 days of stopping an MAOI intended to treat psychiatric disorders.
Watch for symptoms of serotonin syndrome such as agitation, hallucinations, rapid heart rate, sweating, tremor, rigidity, nausea, vomiting, or diarrhea.
Do not stop escitalopram suddenly unless instructed; the label recommends gradually reducing the dose when possible.
Report any new or worsening suicidal thoughts, behavior changes, or unusual worsening of mood right away.
Tell your healthcare provider if you have a history of seizures, bipolar disorder, mania, or hypomania.
Be aware that escitalopram may increase bleeding risk when used with aspirin, NSAIDs, warfarin, or other anticoagulants.
Use caution with driving or operating machinery until you know how escitalopram affects you.
Possible side effects reported in studies included insomnia, nausea, increased sweating, fatigue, somnolence, decreased libido, and ejaculation disorder.
