Adverse Reactions

The most common adverse reactions (≥10%) with estradiol are: headache, breast tenderness, nasopharyngitis, sinusitis, sinus headache, upper respiratory tract infection, back pain, depression, and irregular vaginal bleeding or spotting. (6.1)

Drug Interactions

Inducers and/or inhibitors of CYP3A4 may affect estrogen drug metabolism and decrease or increase the estrogen plasma concentration. (7)

Overview

Estradiol transdermal system is an estrogen patch used for several menopause-related and hormone-related conditions. It is indicated for treatment of moderate to severe hot flashes, moderate to severe vulvar and vaginal atrophy symptoms due to menopause, hypoestrogenism due to hypogonadism, castration, or primary ovarian failure, and for prevention of postmenopausal osteoporosis.

The label says to use the lowest effective dose for the shortest time needed. For women with a uterus, a progestogen is generally considered along with estrogen to reduce the risk of endometrial cancer. For prevention of osteoporosis alone, non-estrogen medicines should be considered first.

This product carries important warnings about increased risks of endometrial cancer, stroke, blood clots, probable dementia in women 65 years and older in WHI studies, and breast cancer with certain estrogen plus progestin therapy. It should not be used to prevent heart disease or dementia.

The patch is applied to a clean, dry area of the lower abdomen or buttocks, not the breasts, and is replaced twice weekly. If a patch falls off or is forgotten, the label instructs applying a patch as soon as possible and continuing the original schedule.

  • Apply the patch to a clean, dry area of the lower abdomen, buttocks, or trunk, and do not apply it to the breasts.
  • Replace the patch twice weekly.
  • Rotate application sites and wait at least 1 week before using the same site again.
  • Do not use the patch on oily, damaged, or irritated skin, and avoid the waistline.
  • Press the patch firmly in place for about 10 seconds, especially around the edges.
  • If a patch falls off, reapply the same patch or use a new one at another site and keep the original schedule.
  • If a dose is forgotten, apply a new patch as soon as possible and continue the original schedule.
  • Use the lowest effective dose for the shortest duration consistent with treatment goals and risks.
  • If you have a uterus, a progestogen is generally considered with estrogen therapy to reduce the risk of endometrial cancer.
  • Do not use this medicine for prevention of cardiovascular disease or dementia.
