- Angioedema: Discontinue lisinopril, provide appropriate therapy and monitor until resolved (5.2)
- Renal impairment: Monitor renal function periodically (5.3)
- Hypotension: Patients with other heart or renal diseases have increased risk, monitor blood pressure after initiation (5.4)
- Hyperkalemia: Monitor serum potassium periodically (5.5)
- Cholestatic jaundice and hepatic failure: Monitor for jaundice or signs of liver failure (5.6)

Overview
Lisinopril is an ACE inhibitor used to treat high blood pressure in adults and in pediatric patients 6 years of age and older. It is also used as add-on treatment for systolic heart failure and to reduce the risk of death in certain patients within 24 hours of an acute myocardial infarction.
For hypertension in adults, the usual starting dose is 10 mg once daily, with adjustment based on blood pressure response. Different starting doses are used for people taking diuretics, for some children, for heart failure, for acute myocardial infarction, and for patients with kidney impairment.
The label includes a boxed warning for fetal toxicity. Lisinopril should be stopped as soon as pregnancy is detected. It is also contraindicated with sacubitril-containing products, in people with a history of ACE inhibitor-related angioedema or hereditary/idiopathic angioedema, and with aliskiren in patients with diabetes.

Key Counseling Points
- Take lisinopril exactly as prescribed by your clinician.
- This medicine is used for high blood pressure, heart failure, or after certain heart attacks.
- Do not use lisinopril with sacubitril-containing products; the label says not to take it within 36 hours of switching to or from sacubitril/valsartan.
- Get medical help right away for swelling of the face, lips, tongue, throat, or trouble breathing, which may be signs of angioedema.
- Tell your clinician if you become pregnant; the label says lisinopril can cause fetal harm and should be discontinued as soon as possible when pregnancy is detected.
- Lisinopril can cause low blood pressure, especially when starting treatment or if you take a diuretic.
- Your clinician may check kidney function and blood potassium during treatment.
- The label lists cough, dizziness, headache, and hypotension among common adverse reactions.
- Avoid using aliskiren with lisinopril if you have diabetes, as stated in the label.
- Tell your clinician about all medicines you take, including diuretics, NSAIDs, lithium, potassium supplements, and potassium-sparing diuretics.
