Suicidality and Antidepressant Drugs
Antidepressants increased the risk compared to placebo of suicidal thinking and behavior (suicidality) in children, adolescents, and young adults in short-term studies of major depressive disorder (MDD) and other psychiatric disorders. Anyone considering the use of nortriptyline hydrochloride or any other antidepressant in a child, adolescent, or young adult must balance this risk with the clinical need. Short-term studies did not show an increase in the risk of suicidality with antidepressants compared to placebo in adults beyond age 24; there was a reduction in risk with antidepressants compared to placebo in adults aged 65 and older. Depression and certain other psychiatric disorders are themselves associated with increases in the risk of suicide. Patients of all ages who are started on antidepressant therapy should be monitored appropriately and observed closely for clinical worsening, suicidality, or unusual changes in behavior. Families and caregivers should be advised of the need for close observation and communication with the prescriber. Nortriptyline hydrochloride is not approved for use in pediatric patients (see WARNINGS, Clinical Worsening and Suicide Risk; PRECAUTIONS, Information for Patients; and PRECAUTIONS, Pediatric Use).

Overview
Nortriptyline Hydrochloride Capsules, USP are a tricyclic antidepressant used for the relief of symptoms of depression. The label says endogenous depressions are more likely to be helped than other depressive states.
The medicine carries a boxed warning about suicidal thoughts and behavior with antidepressants, especially in children, adolescents, and young adults. Patients of all ages should be watched closely for worsening depression, unusual behavior changes, or suicidality, especially when treatment starts or the dose changes.
The label also warns about important safety issues, including serotonin syndrome, heart-related effects, angle-closure glaucoma, and interaction risks with MAO inhibitors, alcohol, and certain other medicines. Nortriptyline hydrochloride is not approved for pediatric use.
Storage Information
storage of catecholamines. Protect the patient's airway, establish an intravenous line and initiate gastric decontamination. Dispense in a tight container, as defined in the USP, with a child-resistant closure. Store at 20° to 25°C (68° to 77°F) [See USP Controlled Room Temperature].

- Take this medicine exactly as prescribed.
- Watch for worsening depression, unusual behavior changes, or suicidal thoughts, especially when starting treatment or changing the dose.
- Family members or caregivers should also watch for sudden changes in mood, behavior, agitation, irritability, or sleep.
- Do not use with MAO inhibitors; the label lists serious serotonin syndrome risk with these combinations.
- Tell the prescriber about all other medicines and supplements, especially serotonergic drugs and drugs that affect CYP2D6.
- Use caution with alcohol because the label says alcohol can have an exaggerated effect.
- This medicine may impair thinking or physical abilities needed for driving or operating machinery.
- Report symptoms that may suggest serotonin syndrome, such as agitation, hallucinations, rapid heart rate, dizziness, sweating, tremor, stiffness, or diarrhea.
- People with heart disease, seizure history, urinary retention, glaucoma risk, or bipolar disorder need special caution according to the label.
- If overdose is suspected, the label advises urgent medical attention and poison control guidance.
