Suicidality and Antidepressant Drugs

Antidepressants increased the risk compared to placebo of suicidal thinking and behavior (suicidality) in children, adolescents, and young adults in short-term studies of Major Depressive Disorder (MDD) and other psychiatric disorders. Anyone considering the use of venlafaxine tablets or any other antidepressant in a child, adolescent, or young adult must balance this risk with the clinical need. Short-term studies did not show an increase in the risk of suicidality with antidepressants compared to placebo in adults beyond age 24; there was a reduction in risk with antidepressants compared to placebo in adults aged 65 and older. Depression and certain other psychiatric disorders are themselves associated with increases in the risk of suicide. Patients of all ages who are started on antidepressant therapy should be monitored appropriately and observed closely for clinical worsening, suicidality, or unusual changes in behavior. Families and caregivers should be advised of the need for close observation and communication with the prescriber. Venlafaxine tablets are not approved for use in pediatric patients. Overview

Venlafaxine tablets, USP are a prescription medicine used to treat major depressive disorder in adults. The label says the medicine was studied in short-term trials and in longer-term relapse-prevention studies.

The label includes an important warning about suicidal thinking and behavior in children, adolescents, and young adults taking antidepressants. Venlafaxine tablets are not approved for use in pediatric patients.

The label also warns about serotonin syndrome, increased blood pressure, angle-closure glaucoma risk, and symptoms that may happen when the medicine is stopped too quickly. Patients should be monitored for worsening depression, unusual behavior changes, and other warning signs.

Some people may have nausea, anxiety, insomnia, sweating, dizziness, weight loss, or sexual side effects. The label says dose changes or stopping treatment should be done gradually whenever possible.

Key Counseling Points

  • Take venlafaxine exactly as prescribed.
  • Do not stop taking it suddenly unless your prescriber tells you to.
  • Tell a health care provider right away about worsening depression, suicidal thoughts, unusual behavior changes, or severe agitation.
  • Know the symptoms of serotonin syndrome, such as agitation, hallucinations, fever, fast heart rate, stiffness, tremor, vomiting, or diarrhea, and seek help if they occur.
  • Tell your prescriber about all medicines and supplements you take, especially MAOIs, linezolid, intravenous methylene blue, triptans, tramadol, lithium, buspirone, tryptophan, St. Johnu2019s Wort, and other antidepressants.
  • Have blood pressure checked regularly while taking venlafaxine.
  • Tell your health care provider if you have a history of bipolar disorder, glaucoma, liver disease, or kidney disease.
  • Report any bothersome sexual side effects to your prescriber.
  • If the medicine is being stopped, the dose should usually be reduced gradually to help lower the risk of discontinuation symptoms.
  • Keep follow-up appointments so your response and side effects can be monitored.
