Suicidality and Antidepressant Drugs

Antidepressants increased the risk compared to placebo of suicidal thinking and behavior (suicidality) in children, adolescents, and young adults in short-term studies of Major Depressive Disorder (MDD) and other psychiatric disorders. Anyone considering the use of venlafaxine tablets or any other antidepressant in a child, adolescent, or young adult must balance this risk with the clinical need. Short-term studies did not show an increase in the risk of suicidality with antidepressants compared to placebo in adults beyond age 24; there was a reduction in risk with antidepressants compared to placebo in adults aged 65 and older. Depression and certain other psychiatric disorders are themselves associated with increases in the risk of suicide. Patients of all ages who are started on antidepressant therapy should be monitored appropriately and observed closely for clinical worsening, suicidality, or unusual changes in behavior. Families and caregivers should be advised of the need for close observation and communication with the prescriber. Venlafaxine tablets are not approved for use in pediatric patients.   • Take venlafaxine exactly as prescribed.
  • Do not use venlafaxine with MAO inhibitors, and do not start it if you are taking linezolid or intravenous methylene blue.
  • Watch for worsening depression, suicidal thoughts, unusual behavior changes, agitation, irritability, or insomnia, especially early in treatment or when the dose changes.
  • Get medical help right away if symptoms of serotonin syndrome occur, such as agitation, hallucinations, fast heartbeat, fever, stiff muscles, tremor, diarrhea, or vomiting.
  • Do not stop venlafaxine suddenly unless directed; the label recommends gradual dose reduction when possible.
  • Tell your prescriber if you have high blood pressure, because venlafaxine can cause sustained increases in blood pressure and regular monitoring is recommended.
  • Tell your prescriber if you have a history of narrow-angle eye problems, since antidepressant-related pupil dilation may trigger an angle-closure attack.
  • Report sexual side effects if they occur, because the label notes decreased libido, delayed orgasm, erectile dysfunction, or delayed ejaculation can happen with SNRIs.
  • Let your prescriber know if you notice anxiety, nervousness, insomnia, dizziness, nausea, sweating, or other symptoms after a dose change or stopping the medicine.
  • Use caution with other serotonergic medicines or supplements, including certain antidepressants, triptans, tramadol, lithium, buspirone, fentanyl, methadone, meperidine, amphetamines, tryptophan, and St. John's Wort.
