Journal Article Summary
The article investigates the implementation of substance use screening in rural federally-qualified health center (FQHC) clinics, highlighting the high rates of unhealthy alcohol and cannabis use among adult primary care patients. This topic is significant because rural communities often face unique challenges related to substance use, including limited access to treatment and higher rates of substance-related harms. By exploring the feasibility of integrating electronic health record (EHR)-based screening in these settings, the study aims to improve the identification and management of substance use issues among patients who may otherwise go unrecognized.
In this study, researchers collaborated with three FQHC clinics in Maine to implement the Tobacco, Alcohol, Prescription Medication, and Other Substance (TAPS) Tool for screening. Over the first year, 3,749 patients completed the screening, representing 93.4% of those eligible during annual preventive care visits. The findings revealed that 30.4% of patients screened positive for moderate-high risk alcohol use, 10.8% for cannabis use, and 14.6% for tobacco use. However, only 17.4% of those who screened positive received documented counseling, indicating a gap in follow-up care for patients identified as at risk.
The study has several limitations, including its focus on a single FQHC, which may not represent all rural practices, and the potential for underreporting of substance use due to stigma. Patients should discuss their substance use openly with healthcare professionals, especially if they have concerns about alcohol or drug use. This conversation can help ensure appropriate screening and support, as well as access to necessary interventions. The findings underscore the importance of integrating substance use screening into routine care, particularly in rural settings where such issues may be prevalent yet under-addressed.
