Opill works best when it is taken exactly as directed. For a daily progestin-only birth control pill, timing matters. Patients should take one tablet at the same time every single day, even during bleeding, spotting, or a menstrual period.
The basic daily routine
- Take one tablet every day.
- Take it at the same time each day.
- Do not skip tablets.
- Do not take a break between packs.
- Start the next pack the day after finishing the current pack.
Because Opill is a progestin-only pill, being late by more than three hours is treated differently than simply taking it a little later than usual. Patients should build a habit around a predictable time, such as after brushing teeth, with breakfast, or before bed.

When backup protection is needed
The label explains that Opill takes 48 hours to start working when beginning the first pack. A condom or another barrier method should be used every time you have sex for the first two days after starting. Backup protection is also needed for 48 hours after a missed pill, a pill taken more than three hours late, or vomiting or severe diarrhea within four hours of taking a tablet.
Do not stop just because your period changes
Some people experience spotting, lighter bleeding, heavier bleeding, shorter periods, longer periods, or no period while using progestin-only contraception. Do not stop or skip tablets just because bleeding patterns change. If bleeding is severe, concerning, or persistent, seek medical guidance.
