Journal Article Summary
The article investigates the long-term effects of escitalopram and exercise on anxiety in patients with coronary heart disease (CHD). Anxiety is a prevalent issue among individuals with CHD and can lead to worse health outcomes. Previous research indicated that escitalopram, a selective serotonin reuptake inhibitor, was more effective than exercise or a placebo in reducing anxiety over a 12-week period. This study aims to determine whether the benefits of these treatments persist for six months after the initial treatment period.
In the UNWIND trial, 128 participants with CHD and significant anxiety were randomly assigned to receive either escitalopram, exercise, or a placebo for 12 weeks. After the treatment, 120 participants were followed for six months to assess anxiety levels using the Hospital Anxiety and Depression Scale-Anxiety (HADS-A). The findings revealed that those who received escitalopram continued to have lower anxiety scores compared to the exercise and placebo groups. While participants in the exercise group showed improved physical capacity, they did not experience significant reductions in anxiety or depression compared to those in the placebo group.
The study has some limitations, including a small sample size and a low number of major adverse cardiac events, which may affect the reliability of the findings. Additionally, many participants continued taking escitalopram during the follow-up, which could influence the outcomes. Patients should consult their healthcare providers about the best treatment options for anxiety, especially if they have CHD, as the study suggests that while escitalopram is effective, exercise alone may not adequately address anxiety symptoms.
