Allopurinol Tablets, for oral use

Initial U.S. Approval: 1966

Indications And Usage

Allopurinol Tablets are xanthine oxidase inhibitor indicated for the management of:

Adult patients with signs and symptoms of primary or secondary gout (acute attacks, tophi, joint destruction, uric acid lithiasis, and/or nephropathy) (1)

Adult and pediatric patients with leukemia, lymphoma and solid tumor malignancies who are receiving cancer therapy which causes elevations of serum and urinary uric acid levels (1)

Adult patients with recurrent calcium oxalate calculi whose daily uric acid excretion exceeds 800 mg/day in male patients and 750 mg/day in female patients, despite lifestyle changes (1)

Limitations of Use

Allopurinol Tablets are not recommended for the treatment of asymptomatic hyperuricemia. (1)

Known hypersensitivity to allopurinol or to any of the ingredients of allopurinol tablets. (4)

Warnings And Precautions

  • Skin Rash and Hypersensitivity: Allopurinol has been associated with serious and sometimes fatal dermatological reactions. Discontinue allopurinol tablets at the first appearance of skin rash or other signs of hypersensitivity reaction. (5.1)
  • Gout Flares: May occur during initiation of treatment. Concurrent prophylactic treatment with colchicine or anti-inflammatory agents is recommended. (5.2)
  • Nephrotoxicity: Allopurinol may affect kidney function. Patients with decreased kidney function require lower doses of allopurinol tablets. (5.3)
  • Hepatoxicity: Cases of reversible hepatotoxicity have occurred. If signs and symptoms of hepatotoxicity develop, evaluate liver function. (5.4)
  • Myelosuppression: Bone marrow suppression has been reported with allopurinol. (5.5)
  • Potential Effect on Driving and Use of Machinery: Drowsiness, somnolence and dizziness have been reported in patients taking allopurinol tablets. (5.6)

Adverse Reactions

Most common adverse reactions (incidence > 1%) are nausea, diarrhea, and increase in liver function tests. (6)

Overview

Allopurinol tablets are a xanthine oxidase inhibitor used to manage gout in adults, certain cancer-therapy-related increases in uric acid in adults and children, and recurrent calcium oxalate stones in some adults with high uric acid excretion. They are not recommended for asymptomatic hyperuricemia.

The label says treatment for gout should start with testing such as serum uric acid, blood counts, chemistry panel, liver function, and kidney function. For gout, prophylaxis against flares is recommended when treatment begins, and gout flares may still happen early in therapy.

Important safety warnings include serious skin reactions and hypersensitivity, kidney problems, liver problems, and bone marrow suppression. Drowsiness, sleepiness, and dizziness have been reported, so patients should be careful with driving or machinery until they know how the medicine affects them.

Storage Information

Storage and Handling  Store at 20°C to 25°C (USP Controlled Room Temperature) (68°F to 77°F) in a dry place. Dispense in a tight container as defined in the USP.

  • Take allopurinol exactly as prescribed.
  • For gout, treatment may start low and be increased gradually.
  • If a dose is missed, do not double the next dose.
  • The tablets are generally better tolerated if taken after meals.
  • Drink enough fluids as directed in the label to support urine output.
  • A gout flare can happen when treatment is started; the label says allopurinol does not need to be stopped if this occurs.
  • Stop allopurinol and seek medical attention promptly if a skin rash or other signs of hypersensitivity appear.
  • Tell the prescriber about kidney disease, liver problems, or a history of low blood counts.
  • Use caution with driving or other dangerous activities if you feel drowsy, dizzy, or sleepy.
  • Tell the prescriber about all other medicines, including mercaptopurine, azathioprine, warfarin, theophylline, capecitabine, and antibiotics such as ampicillin or amoxicillin.
