  • Angioedema: Discontinue lisinopril, provide appropriate therapy and monitor until resolved (5.2)
  • Renal impairment: Monitor renal function periodically (5.3)
  • Hypotension: Patients with other heart or renal diseases have increased risk, monitor blood pressure after initiation (5.4)
  • Hyperkalemia: Monitor serum potassium periodically (5.5)
  • Cholestatic jaundice and hepatic failure: Monitor for jaundice or signs of liver failure (5.6)
  • Angioedema: Discontinue lisinopril, provide appropriate therapy and monitor until resolved (5.2)
  • Renal impairment: Monitor renal function periodically (5.3)
  • Hypotension: Patients with other heart or renal diseases have increased risk, monitor blood pressure after initiation (5.4)
  • Hyperkalemia: Monitor serum potassium periodically (5.5)

Overview

Lisinopril tablets are an ACE inhibitor used to treat high blood pressure in adults and in children 6 years of age and older. The tablets are also indicated as add-on treatment for systolic heart failure and to reduce the risk of death after a hemodynamically stable acute myocardial infarction within 24 hours of onset.

The label includes a boxed warning about fetal toxicity. If pregnancy is detected, lisinopril should be discontinued as soon as possible. Drugs that act directly on the renin-angiotensin system can cause injury and death to the developing fetus.

Lisinopril is not to be used with sacubitril/valsartan within 36 hours, and it is contraindicated in people with a history of ACE inhibitor-related angioedema, hereditary or idiopathic angioedema, or in patients with diabetes who are receiving aliskiren. The label also warns about low blood pressure, kidney problems, high potassium, and liver injury.

Storage Store at 20 ºC to 25ºC (68ºF to 77ºF) [see USP controlled room temperature].

  • Take lisinopril exactly as prescribed; doses depend on the condition being treated and kidney function.
  • Tell the patient to seek urgent help for swelling of the face, lips, tongue, throat, or trouble breathing.
  • Advise the patient that lisinopril can cause dizziness or low blood pressure, especially when starting treatment or after a dose increase.
  • Remind the patient that blood pressure, kidney function, and potassium may need periodic monitoring.
  • Do not use lisinopril with sacubitril/valsartan within 36 hours, and do not combine it with aliskiren in patients with diabetes.
  • Tell the patient to report pregnancy right away; the label says to stop lisinopril as soon as possible if pregnancy is detected.
  • Warn the patient that NSAIDs may worsen kidney function and reduce blood pressure control with lisinopril.
  • Mention that common adverse reactions listed for hypertension include headache, dizziness, and cough.
