Journal Article Summary

The article investigates the timing of administering esomeprazole alongside dual antiplatelet therapy (DAPT) in patients with acute coronary syndrome (ACS) or acute ischemic stroke (AIS). DAPT, which combines clopidogrel and aspirin, is crucial for preventing further ischemic events in these patients. However, concerns exist regarding potential interactions between clopidogrel and proton pump inhibitors (PPIs) like esomeprazole, which may affect the effectiveness of clopidogrel. This study aims to clarify whether taking these medications simultaneously or with a time interval between them impacts clinical outcomes, particularly major adverse cardiocerebrovascular events (MACCEs) and bleeding risks.

The study included 3,600 patients who had experienced ACS or AIS within the past month and were expected to continue DAPT for at least six months. Participants were divided into two groups: one receiving esomeprazole and DAPT concurrently, and the other with a 12-hour interval between the two medications. Over a follow-up period of about six months, the occurrence of MACCEs was similar in both groups, with 0.9% in the interval group and 1.8% in the concurrent group. Additionally, there were no significant differences in bleeding risks between the two groups, suggesting that the timing of esomeprazole administration does not significantly affect the safety or efficacy of DAPT.

Despite the findings, the study has limitations, including a small sample size in the interval group and a lack of individual platelet function test data, which could influence the results. The observational nature of the study means it could not control for all confounding factors, and the results may not be generalizable beyond the Korean population where the study was conducted. Patients should discuss their medication regimens with healthcare professionals, especially regarding the timing of esomeprazole and DAPT, to ensure optimal treatment and safety based on their individual health needs.
